How to enrol your baby in Medicare

Find out about enrolling your baby in Medicare when you claim a payment or using Medicare through myGov.

You can enrol your baby in Medicare when you claim a Centrelink family payment.

Use your Newborn Child Declaration form when you claim the Centrelink payment. On this form you can enrol your baby in Medicare and create a My Health Record for them.

Make sure the proof of birth on the back page is complete.

The form is in the Parent Pack your hospital gives you when your baby was born.

Once your child's been enrolled, you'll get a new Medicare card. We'll send you the physical one in the mail, but you can also access a digital version in your myGov app.

If you’re not claiming a Centrelink payment, you can enrol your baby in your Medicare online account through myGov. You’ll need proof of your baby’s birth, such as:

  • the back page of the Newborn Child Declaration form
  • their birth certificate
  • other Australian Government documents that verify the child’s name and date of birth
  • a letter from the doctor or midwife who attended the birth.

If your Medicare online account is linked to myGov, sign in now to enrol your baby.

Steps to enrol online

If your Medicare online account is linked in myGov, you can submit the form by following these steps:

  1. Sign in to myGov and select Medicare.
  2. Select Services, then Enrol a newborn.
  3. Confirm your and your baby’s details.
  4. Upload your supporting documents and select Next.
  5. Choose to add your baby to another person’s Medicare card.
  6. Review and submit the enrolment.
