Managing your health information

Use My Health Record or the 1800MEDICARE app, get Medicare records and request health information.

My Health Record

My Health Record is a safe and secure place to keep your health information. Access My Health Record through myGov to:

  • view your medicines
  • manage your medical information
  • get your complete immunisation history statement
  • view your pathology and diagnostic imaging reports
  • add or update your personal information
  • add or update your emergency contact details.

You can also use the 1800MEDICARE app to access this information. Find out more about the 1800MEDICARE app on the Australian Digital Health Agency website.

Immunisation and other Medicare records

Use your Medicare online account through myGov to view your:

  • immunisation history statement
  • care plan history
  • organ donation decision.

If you can’t get your statement online, find other ways to get your immunisation history statement on the Services Australia website.

For privacy reasons, children aged 14 years or older can get their own immunisation history statement.

Request information from your health provider

To request access to your health information, contact your health service provider. Only you or a person you’ve authorised can make the request.

Find out more about requesting health information on the Office of the Australian Information Commissioner website.
